Next, enter your mobile number carefully, as you will receive an OTP for verification. Although postpaid numbers are preferred, prepaid numbers also work without any issue. You can also add an emergency contact number, preferably of a family member.

After this, you need to enter your address and medical details. Select your state, district, and the authorized hospital from where your medical certificate was issued.

Upload your photograph in JPG/JPEG format and your medical certificate in PDF format. Ensure that both files are under 1 MB. Also, enter the correct issue date of your medical certificate.
